This permit must be posted at the confined space at all times during the actual entry. When the entry is complete, the permit will be filed at the jobsite.

OPERATION AND MAINTENANCE POLICY

1. Attend Austin Commercial’s Orientation, schedule with superintendent (dates and times for English and Spanish to be set by project).
2. All employees must have passed a drug screen within 30 days of starting work at the site. Need letter with employee name and employee number stating drug test was negative and signed by company officer.
